• Student Feedback & Complaint Form

  • Students who wish to provide feedback or make a complaint regarding actions or decisions of the University may submit their feedback or complaint below. Students may also directly provide feedback or pursue their complaint with the specific University office or department that has oversight of their concerns. Please see below for resources related to various areas that might be of concern. 

    Once submitted, pertinent information will be forwarded to the University office or department that would be responsible for reviewing and addressing the complaint. If not submitted anonymously, a response documenting the receipt and review of the complaint or feedback will be provided to the student.

    The Vice Chancellor for Student Affairs will retain complaints submitted pursuant to its document retention policy. In addition, the Vice Chancellor for Student Affairs will review the complaints submitted through the form below on an annual basis to evaluate and assess trends as well as areas of improvement that may be made by the university.

    We would also like to hear from students who are particularly pleased with a certain aspect of their campus life or who have had an exceptional experience that they’d like to share. This same form can be submitted to provide a compliment or note of appreciation.
