  • Note regarding lock requests: 

    For students with an NDE (Homebase) lock: please review the NDE Lock Maintenance Guide.

    For students with a Safety Key or card swipe lock: Before you complete this form, we ask that you create a new key card using the kiosk. Often, the issue is that the strip for the key card is scratched up and is responsible for the issue.

    Additionally, while filling out this form, we highly recommend you test your card on the door and observe what happens. It is important for you to be as specific as possible; without such details, your door issue may not be resolved. Lastly, please allow 3 business days to pass before you submit a second request regarding your door issue.

    In the case your request is an emergency, please contact your RA or RCD by email or phone to make sure your request is handled accordingly. If it is after hours, please contact WUPD at (314) 935-5555.

    Note regarding technology requests: If you are requesting a repair for a broken telephone port or network port, please submit your request directly to Student Technology Services.

  • Are you or anyone in your suite/apartment currently being quarantined or isolated for COVID-19?*
  • Are you or anyone living in the suite/apartment or working directly in the office repair location currently experiencing fever, chills, new or worsening cough, trouble breathing, unexplained body aches, sore throat different from seasonal allergies, new or worsening diarrhea, new loss of taste or smell?*
